Forehead wrinkles are caused by repeated movement of the frontalis muscle, which lifts the eyebrows and creates horizontal lines across the forehead. Dr Kynan performs anti-wrinkle injections to carefully relax this muscle — reducing visible lines while preserving natural facial expression.
How Forehead Wrinkles Form
As the skin puts pressure on the underlying fat pad, it eventually causes thinning and gradual destruction of the fat, allowing the skin to drop to the layer below — resulting in a crease or permanent wrinkle.
Stress Line
A line visible through the day under close inspection, but which generally recovers overnight when pressure is removed from the lower fat layers.
Dynamic Wrinkle
Lines that appear with movement and begin persisting at rest. The optimal time to begin preventative treatment — before structural fat loss occurs.
Permanent Wrinkle
Once wrinkles become permanent, neuromodulators cannot undo them — but they can prevent worsening and slow further progression.
Typical Dose
10–20 units; often combined with frown treatment to maintain brow support
When to Start
Most patients begin in their late 20s or early 30s, preventatively or once dynamic wrinkles persist at rest
Brow Lift Effect
When appropriately performed, treatment can create a subtle chemical brow lift, opening the eyes naturally
Treatment Approach
Treatment requires precise injection placement and depth. Injections are performed superficially within the frontalis muscle, with higher forehead placement preferred to minimise the risk of brow ptosis. Injection strategy is tailored to individual anatomy, brow position, and muscle strength.
"Careful assessment by an experienced medical practitioner is essential — the difference between a lifted, refreshed result and an unnatural one is measured in millimetres."
When treating the frontalis, a deliberate injection plan is essential. Key considerations include baseline brow position, existing asymmetry, frontalis strength, patient age, and the functional impact of reduced brow elevation.
Dr Kynan follows a "good, better, best" dosing philosophy, preferring to start conservatively and titrate at review rather than over-treat at the initial session.
A zone immediately superior to the brow is intentionally left untreated, achieving a subtle natural brow lift and preserving movement to assist with makeup application.
Injections are continued at approximately 1.5 cm intervals across the forehead, including above the brow until the lateral aponeurosis is reached — maintaining a flatter, more masculine brow contour.
The goal is a strong, horizontal brow shape rather than the arched lift preferred in female treatment.
Injection Depth — Critical Technique
Advance to gentle periosteal contact, then withdraw approximately 1 mm before injecting. This deposits toxin into the belly of the frontalis while avoiding superficial hydrodissection — improving both comfort and clinical effect.
Aftercare Instructions
Following your forehead anti-wrinkle treatment, please observe the following guidelines carefully to protect your result.
Remain upright for four hours following treatment and avoid lying down while the protein settles.
Delay facials or invasive dermal treatments like microdermabrasion or skin needling for two weeks.
Keep your face moving — frown, laugh, smile normally to help the protein absorb in the targeted muscle.
Avoid alcohol and painkillers before and after treatment as they thin the blood and increase bruising risk.
Exercise less than 12 hours after treatment. Avoid inverted poses (yoga, pilates, barre) while the injection is settling.
Expose skin to excessive heat or cold — saunas, spas, steam rooms, hot yoga, ice baths or extreme cold.
Massage or apply pressure to the face. When cleansing and applying skincare or makeup, use gentle strokes only.
