Forehead Wrinkle Treatment on the Gold Coast

Forehead wrinkles are caused by repeated movement of the frontalis muscle, which lifts the eyebrows and creates horizontal lines across the forehead. As the skin puts pressure on the underlying fat pad, eventually it causes the thinning and eventual destruction of the fat, allowing the skin to drop down to the layer below, resulting in a crease of permanent wrinkle. The first sign of a wrinkle is a stress line. This is a line which may be visible through the day under close inspection, but generally recovers during the night when the stress isnt being applied to the lower fat layers. Stress lines over time, form more permanent wrinkles. Once wrinkles become permanent, neuromodulators cannot be used to undo them, but they can prevent worsening.

Treatment requires precise injection placement and depth. Injections are performed superficially within the frontalis muscle, with higher forehead placement preferred to minimise the risk of brow ptosis (eyebrow drooping). Injection strategy is tailored to individual anatomy, brow position, and muscle strength to ensure a safe and balanced result.

Most patients begin treatment in their late 20s or early 30s, either preventatively or once dynamic wrinkles persist at rest. Typical forehead dosing varies, commonly 10–20 units, and is often combined with frown muscle treatment to maintain brow support.

When appropriately performed, treatment can create a subtle chemical brow lift, opening the eyes while maintaining a natural appearance. Careful assessment by an experienced medical practitioner is essential for optimal outcomes.

For Injectors: Forehead Anti-Wrinkle Injection Principles

When treating the frontalis, a deliberate injection plan is essential. Key considerations—particularly in female patients—include baseline brow position, existing asymmetry, frontalis strength, patient age, and the functional impact of reduced brow elevation. Practical issues such as brow heaviness and ease of eye-makeup application (especially eyeliner) should always be discussed pre-treatment.

Dr Kynan follows a “good, better, best” dosing philosophy, preferring to start conservatively and titrate at review rather than over-treat at the initial session.

Female Injection Pattern: “Cat’s Ear Sparing” Technique

In female patients, the cat’s ear sparing technique is employed. A zone immediately superior to the brow is intentionally left untreated. This achieves two outcomes:

1. A subtle, natural brow lift

2. Preservation of movement to assist with makeup application

This zone is mapped by drawing a line from the lateral nasal alar through the pupil to the hairline, then extending two lines back to the medial and lateral brow. Injection should remain at least 3 cm above the orbital rim in all patients.

If residual “wifi lines” appear within the cat’s ear zone, a single unit may be added superiorly at review.

Male Injection Pattern

In male patients, injections are continued at ~1.5 cm intervals across the forehead, including above the brow until the lateral aponeurosis is reached, maintaining a flatter, more masculine brow contour.

Injection Depth

Depth is critical. Advance to gentle periosteal contact, then withdraw approximately 1 mm before injecting. This deposits toxin into the belly of the frontalis while avoiding superficial hydrodissection, improving comfort and clinical effect.

Forehead Wrinkles

DO:

  • Remain upright for four hours following your treatment and avoid lying down while the protein settles into place

  • Delay any facials or invasive dermal treatments like microdermabrasion, skin needling, or dermal rolling for two weeks after anti-wrinkle injections

  • Keep your face moving. Frown, laugh, smile as you would normally as this will help the protein to be more quickly absorbed in the targeted muscle.

  • Avoid alcohol and painkillers before and after treatment as they tend to thin the blood and can make you prone to bruising

DON’T:

  • Exercise less than 12 hours after your treatment. We also recommend avoiding any exercise that involves inverted poses (eg yoga, pilates, barre etc) while the anti-wrinkle injection is settling into place during this time

  • Expose your skin to excessive heat or cold including saunas, spas, steam rooms, hot yoga, ice baths or skiing. Changes in temperature can lead to you touching or rubbing your face more which can put unwanted pressure on the treated muscles, risking dispersion of the protein into unwanted areas

  • Massage or apply pressure to the face which can affect how it settles in a particular area. When cleansing the skin and applying skin care and makeup use gentle strokes
